Abstract
In this report a reference model for Disease Surveillance System (DSS) is presented. Implementation of this reference model can give solutions to inefficiencies in the health care service due to paper based health record keeping , inaccurate information analysis of the experts due to information not describe as they are and improve the health care services with the usage of Information Technology. The reference model brings up what Disease Surveillance System should be like in order to deliver high quality health care services. The reference model includes three components: * Functional model * Architectural model * Data model Functional model describes the functionality of the system and architectural and data models were designed in such a way that the functionality can be achieved. Integration of GIS mapping, chart generation and reports into Disease Surveillance System can lead to transformational improvements in the features provided by Disease Surveillance System Therefore, the model was designed to use GIS mapping, chart generation and reporting.
